YOUTH Registration
- Day of the week: Mondays
- Dates: June 12 – July 24 (no session July 3) 6 Sessions
- Time: 5-6 pm
- Grade: 4-6 (Sept 2017)
All players must be US Lacrosse Members- If you’re not a member register here online: US Lacrosse Membership
All players must bring a filled out copy of the Liability Waiver and Medical Release Authorization to the first session in order to play
**All games will be held at the Portland Sports Complex
IMPORTANT: ALL REGISTRANTS MUST PRINT, SIGN AND BRING A COPY OF THE LIABILITY WAIVER AND MEDICAL RELEASE FORM WITH THEM TO THE FIRST SESSION. THIS FORM CAN ALSO BE SCANNED AND SENT ELECTRONICALLY TO BOB@LAXPROS.COM
